- For a new name: a sworn affidavit, a newspaper publication and another government ID, plus your marriage certificate or publication if you married.
- To rearrange your names: a government ID, such as a driver’s licence, voter’s card or passport, that already shows them in the new order.
- For an address change: a utility bill, tenancy agreement or bank statement not older than 3 months, and an application letter. NIMC approves address changes case by case.
- For a new phone number after loss or damage: a police report and an affidavit.
- For education: the certificate you obtained and an application letter. For occupation: your offer letter.
- A new passport photograph, if you are changing your photo.
